It sounds like YOU have a PROBLEM!! Try THIS -- DON'T WEAR any underwear for -- oh, A DAY -- and see how YOU FEEL! I do it all the time because I get too hot in the summer. The only problem with that is there is no fabric to soak up persperation, so you will have to experiment. OR - go to your local camping or sporting goods store and get SILK underwear!!! SILK will keep you COOL and also WARM when you need it!! I bought some K underwear for a friend because he was ALWAYS getting a COLD butt. He LOVED the SILK UNDERWEAR!!

I wore tighty-whities for a while after my cord injury. Then I atarted to notice that as my legs were atrophying, my junk was just sliding through the leg holes. I felt like I had the waistline of a grown man, with the legs of a pre teenager. I was very concerned about my scrotum getting tangled in the extra fabric of my undies and getting some sort of circulation disruption. So yeah, commando it is for me. Less material to worry about having create a pressure spot as well. I alwas would find that my drawers would try and make like a thong, and pressure was a concern. So never again will I be ruled by the gods of underwear.

Ok well i am a female so i dont really know much about the male problem but i will say that thongs are not uncomfortable, i wore them before and after my accident and when i could feel that part of my body well honestly you dont have to go around pulling them out of ur butt lol unless well u get pull happy and pull them up there in the first place. from my experience with them i never felt them before my accident when i wore them. But i suggest you try everything once because you will never know what works until you try it.

Regarding caregivers and being embarassed. I have been in the"hands on" medical field for over 20 years. Trust me when I tell you that when things should "pop up" the only one that feels embarassed is you, if you let it. Professional caregivers are just that, Professional. We don't think anything of issues that pop up, including putting your package into your thong, cup, pouch, etc. In my personal opinion you wear what is comfortable for you even if that means letting it all hang out!!
